Dietary and Nutrition

Your family doctor or nurse practitioner can also refer you to the Summerville dietician for nutritional counselling. The dietician will arrange an appointment to meet you here at Apple Hills at a mutually convenient time.

General Medical Care

The family doctors of Apple Hills provide the majority of your medical primary care. The Summerville Family Health Team is complementing these services with nurse practitioners and other health care providers. Our Nurse Practitioners work in collaboration with physicians and registered nurses to care for patients in many different ways.

Some of your care will be provided by other health care providers within the team either by referral or as part of general medical care. Each of the programs listed describes how health care providers support your care.
